Abstract: Niclosamide (NIC), an antihelminthic drug, showed a cardioprotective impact against heart failure in addition to beneficial effects in diabetes-related complications, like nephropathy, neuropathy, and vascular dysfunction. The present study aimed to explore NIC cardioprotective effects in T2DM and its underlying molecular mechanisms related to pyroptosis and the AMPK/FOXO3a autophagy-dependent pathway. Twenty four male Wistar rats were classified into; Control group, DM group: rats in both groups received DMSO (1 ml/kg, i.p), DM+ NIC group: diabetic rats were treated with niclosamide (75 mg/kg/d, i.p), DM + NIC + compound C group: diabetic rats were given compound C (0.2 mg/kg/d, i.p) and niclosamide as previously described. All treatments were administered for eight weeks, beginning after inducing T2DM. Niclosamide injection significantly reduced the LVEDP, while elevated the MABP, LVSP, and -dP/dt max hemodynamic parameters. NIC significantly reduced FBG levels, HOMA-IR, serum TG, TC, HW/BW, CK MB and LDH, downregulated the myocardial gene expression of TGF-β1, Col 1, NLRP-3, IL1β, beclin-1, and LC3-II, while upregulating the p-AMPK/AMPK and p-Foxo3a/Foxo3a ratios. Additionally, NIC improved H&E histopathologic picture, mitigated the myocardial collagen fibers deposition shown by Masson’s trichrome, decreased caspase-1, while increasing p-AMPK immunoreactivity. Furthermore, coadministration of compound C, an AMPK inhibitor, attenuated the beneficial effects of NIC. Niclosamide possesses cardioprotective effects in the HFD/STZ diabetic rat by inhibiting the pyroptosis process, and its effect is mediated mostly through the AMPK/FOXO3a autophagy-dependent pathway.
